A Mental Health Treatment Plan (MHTP) is a support plan you create with your General Practitioner (GP) to help manage your mental health. It’s a written document that outlines:

  • Your current mental health concerns

  • Your goals and priorities for support

  • The treatments, therapies or supports that may help you

  • Referrals to mental health clinicians

Why Have a Treatment Plan?

A Mental Health Treatment Plan helps you access the right supports at the right time. It not only makes care more affordable through Medicare rebates but also ensures your care is coordinated, clear and focused on your goals.

  • Access to Medicare Rebates

    With a valid MHTP, you may be eligible for Medicare rebates for up to 10 sessions per calendar year with a mental health clinician.

  • Holistic and Coordinated Care

    Your GP, clinicians and other providers can work together to support you.

  • Collaborative Approach

    We collaborate with your broader care team (if you’d like) to make sure your supports are coordinated and consistent.

  • Clarity and Direction

    Having your goals and supports written down helps you stay on track and review progress.

How to Get a Mental Health Treatment Plan

Getting a Mental Health Treatment Plan is a straightforward process that begins with a conversation with your GP. Together, you’ll talk about what you’ve been experiencing, identify your goals, and create a plan that connects you with the right supports and professionals.

  • 1. Book a double appointment with your GP

    Let the clinic know you’d like to discuss a Mental Health Treatment Plan; these appointments usually take 30–40 minutes.

  • 2. Talk through your concerns

    Your GP will ask questions about your mental health, what you’ve been experiencing and what supports might help.

  • 3. Create the plan together

    Your GP will document your needs and goals, then provide a referral to a mental health clinician (like one of our clinicians at The Flourish Collective).

  • 4. Bring the plan with you

    When you book with us, simply let us know you have a MHTP, and bring a copy of your referral so we can apply the Medicare rebate to your sessions.

At The Flourish Collective

We aim to make the process as supportive and straightforward as possible. If you’re unsure where to start, we can guide you on how to talk with your GP and what to ask for when requesting a treatment plan.

  • Medicare rebates vary depending on the clinician, with an out-of-pocket cost of approximately $150 per 50-minute session.

  • For clients with a Health Care Card or those who may find the cost financially challenging, we can provide sessions under our Short Term Therapies (STT) program.
